What You Will Do
As an Associate with Via Christi Health, you will have the opportunity to coordinate the overall interdisciplinary plan of care for patients, from admission to discharge.
Create plan for care across the continuum, integrating patient/family preferences and values.
Monitor patient care through assessments, evaluations and/or patient records.
Advocate for resources and removal of barriers.
Maintain ongoing dialog with supervisors and other health providers to ensure effective implementation of health plans.
Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
What You Will Need
Licensed Registered Nurse credentialed from the Kansas Board of Nursing.
BLS Provider credentialed from the American Heart Association (AHA).
Diploma from an accredited school/college of nursing.
Bachelors of Science in Nursing.
5 years of experience in acute care setting.
