Ecumen Hospice is currently seeking a Social Worker, LSW for its newest branch in Owatonna. Ecumen Hospice is a non-profit, Medicare-certified hospice that has been providing life-honoring care in Minnesota for almost 30 years. We serve those who are experiencing a life-limiting condition and the people that are closest to them, through a comprehensive offering of services which include but are not limited to: physician oversight, nursing, social work, spiritual care, hospice aide assistance, music therapy, massage therapy, volunteer services, pet therapy, and bereavement care. This is a 32 hour a week position. Position is eligible for all benefits. This position is responsible for providing direct social work services to residents, patients, extending services to family members and caregivers in need of identified social worker services, and providing them with psychosocial and environmental support. Essential job responsibilities include: Assess the psychosocial status of patients and families/caregivers related to the patient's or resident s condition and environment. Carry out evaluations, including family dynamics, caregiver abilities, and communication patterns, neglect or abuse and plan interventions based as findings warrant. Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses. Maintains clinical records on all patients referred to social work. Assists family and patient in planning for arrangements, financial, legal, and health care decision responsibilities. Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Social Worker Minimum Required Qualifications: Bachelor s Degree in social work Two years of social work experience in a health care setting Licensed Social Worker in good standing in Minnesota Current valid driver s license and proof of vehicle insurance Ability to communicate effectively in both written and verbal formats Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, Resident s Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and MSDS Preferred Qualifications: Master s Degree in social work Experience working with seniors CPR certification Ecumen is an equal opportunity employer.
