Seeking a part-time care manager to be part of a brief critical time intervention (B-CTI) team to provide intensive, community-based care management services to individuals with schizophrenia and chronic medical conditions who have recently been hospitalized for a medical illness and are enrolled in a research study.
Follow study protocol and CTI model as written in the manual and directed by clinical supervisor
--Meet with patients in the hospital prior to discharge to home to facilitate and support the discharge process
--Communicate with the hospital inpatient staff to coordinate discharge planning and aftercare follow-up
--Complete needs assessment to identify barriers and obstacles to successful transition to community care
--Meet with patients in the community upon discharge at least weekly, and routinely for 3 months to support and problem solve around connections with aftercare medical and mental health providers
--Communicate as indicated with medical and mental health providers to coordinate appointments and facilitate patient adherence with treatment recommendations
--Document ALL service planning, collaborative goal setting, linkage, advocacy and mediation activities carried out with and on behalf of the client
--Participate in training and supervision activities with other case manager and supervisors
--Communicate regularly with clinical supervisor and keep track of all field-based activities on a daily basis
--At least 2 years experience working with individuals with severe mental illness in inpatient, outpatient, clinic, or case management, or other support capacity
--Has a firm understanding of the personal challenges and systemic barriers associated with obtaining help for behavioral and other health conditions as well as other areas of need including income, housing and social support
--Knowledgeable about community-based resources for mental health and primary care in the Bronx
--Adept at interdisciplinary team work, including communication with other team members and clinical supervisors
--Experience working with clients in the community and engaging individuals with severe mental illness and substance use disorders
--Computer skills including MS word and excel, and Outlook
